Urgent care and Telecare Response – Empowering Independence, Enhancing Well-Being

An exciting opportunity has arisen within our new Urgent Care and Telecare Response Service for a Senior Social Care Practitioner to join us!

The Urgent Care and Telecare Response Service operates within Wiltshire Support at Home. Our mission is to prevent unnecessary hospital admissions by providing timely and effective care. Where possible we promote independence of our service recipients, ensuring a positive experience that supports both their mental and physical well-being.

As a Senior Social Care practitioner, you will support and mentor Social Care Practitioners while demonstrating your excellent knowledge and ability in supporting adults with care and support needs. Your responsibilities will include assessing and reviewing complex customer needs and outcomes, coordinating the customer/carer journey to identify eligible needs and outcomes.

This role is fast paced so you should be able to manage caseloads effectively and have excellent communication skills to be able to work collaboratively with multi professional teams. You will hold an NVQ Level 4 in Health and Social Care or equivalent or the ability to work towards this qualification and have experience of working within a Health or Social Care setting.
